Services
These Terms and Policies apply to services provided by AmandaMcDPT, LLC d/b/a Unbound Pelvis. Unbound Pelvis provides physical therapy services, pelvic health care, education, and related wellness services. Services may be delivered in person, via telehealth where permitted by law, or through other agreed-upon formats.. Physical therapy services are provided by a licensed physical therapist in accordance with applicable state laws and practice standards. Educational content, courses, and materials offered through Unbound Pelvis are provided for informational purposes only and do not replace individualized medical evaluation or treatment. Participation in services does not guarantee outcomes, as results vary based on individual factors.

Payments & Billing
Payments for services are processed securely through Jane App and Stripe, third-party platforms that support scheduling, practice management, and payment processing. By submitting payment information and completing a purchase, you authorize these providers to process your payment in accordance with their respective terms and conditions and to charge your selected payment method for the amount indicated. Unbound Pelvis does not store, access, or process full credit card information and is not responsible for errors, interruptions, or service disruptions caused by Jane App, Stripe, or other third-party service providers.
Good Faith Estimate (No Surprises Act)
Under the No Surprises Act, healthcare providers are required to provide patients who are uninsured or self-pay with a Good Faith Estimate of expected charges for scheduled services. Unbound Pelvis will provide a Good Faith Estimate upon request and prior to the provision of services when applicable. This estimate outlines the expected costs of services based on information available at the time and is not a guarantee of final charges, as treatment needs may change based on clinical findings or patient response. If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges through the patient–provider dispute resolution process established by the U.S. Department of Health and Human Services. For questions about your Good Faith Estimate or pricing, please contact Unbound Pelvis prior to your appointment.
Refunds, Cancellations & No-Show Policy
Appointments cancelled or rescheduled with less than the required notice may be subject to a cancellation or no-show fee, as outlined at the time of scheduling. Refunds for services are not guaranteed and are considered on a case-by-case basis prior to service delivery. Once an appointment has occurred or services have been rendered, refunds are not available. We understand that unexpected circumstances arise and encourage open communication when concerns occur.
Communications (Email, SMS & Patient Platforms)
If you opt in to receive communications from Unbound Pelvis, we may contact you via email, SMS, or secure patient platforms for scheduling, care coordination, service updates, and educational information.. You may unsubscribe from non-clinical emails at any time using the unsubscribe link. You may opt out of SMS messages by replying STOP. Consent to receive communications is not a condition of purchase. Message and data rates may apply. Wireless carriers are not responsible for delayed or undelivered messages.

Information We Collect
We may collect personal and health-related information necessary to provide physical therapy services, including: name and contact information, health history and intake forms, appointment and treatment records, payment-related information (processed by third-party platforms). Payment information is processed and stored exclusively by Jane App and Stripe and is not stored on Unbound Pelvis servers. Personal information is used solely for treatment, scheduling, billing, communication, and legal or regulatory compliance. We do not sell or share personal information for marketing purposes.
Third-Party Services
We use trusted third-party platforms to support business operations, including scheduling, documentation, payment processing, email delivery, and messaging services. These providers receive only the information necessary to perform their services and are required to protect your data in accordance with their security and privacy standards. Intellectual Property
All website content, educational materials, handouts, programs, and resources provided by Unbound Pelvis are the intellectual property of Unbound Pelvis and may not be reproduced, distributed, or shared without prior written permission.

To the fullest extent permitted by law, Unbound Pelvis is not liable for any indirect, incidental, or consequential damages arising from use of this website, educational materials, or participation in services. All services are provided in good faith and in accordance with applicable professional standards and ethical obligations.
